Skilled nursing at home

Skilled nursing at home is medically necessary nursing care provided by a registered nurse (RN) or licensed practical nurse (LPN) in a patient's residence under a physician's plan of care. Medicare Part A and Part B cover skilled nursing for homebound patients who need part-time intermittent care that is "reasonable and necessary" per CMS guidelines.

Who qualifies for Medicare-covered skilled nursing at home?

A patient qualifies if all of the following apply:

Under the care of a physician with a signed plan of care
Homebound — meaning leaving home requires considerable and taxing effort
Needs part-time skilled nursing, physical therapy, or speech therapy
Receiving care from a Medicare-certified home health agency (Kassy Health is certified — FL AHCA License #299993031)
A face-to-face encounter with the certifying physician occurred within 90 days before or 30 days after the start of care

Visit Structure

What does a skilled nursing visit look like?

A typical visit lasts 45 to 75 minutes. The nurse arrives at a scheduled window, performs the ordered care, documents the visit electronically, and updates the physician on any changes in condition. Family members are encouraged to be present for education-focused visits.

Every Kassy Health patient has one primary case manager — the nurse who knows your history, coordinates your care team, and is your first call with questions between visits.

How often will a nurse visit?

Visit frequency is set by the physician's plan of care. Common patterns:

Post-surgical wound care

2–3 visits per week for 2–4 weeks

IV antibiotic course

Daily visits for the duration of therapy

CHF education & monitoring

1–2 visits per week, tapering as patient stabilizes

Diabetes initiation

Daily for 3–7 days, then weekly

What does skilled nursing cost?

For patients who meet the eligibility criteria above, Medicare Part A covers 100% of approved skilled nursing visits — no copay, no deductible. There is a 20% coinsurance for durable medical equipment (DME) such as wound vacs or infusion pumps.

A physician's order is required for Medicare-covered home health. However, you can call Kassy Health directly and we will contact the physician's office or hospital discharge planner to initiate the referral on your behalf. You do not need to navigate that process alone.

Skilled nursing at home brings a licensed nurse to the patient's residence for intermittent visits — typically 45 to 75 minutes, several times per week. The patient remains at home. A skilled nursing facility (SNF) is a residential care setting where patients stay around the clock. For appropriate diagnoses, home health is equally effective clinically and strongly preferred by most patients and families.

Kassy Health assigns a primary case manager to every patient — that nurse is your consistent clinical contact. While scheduling occasionally requires a backup clinician, continuity is a core commitment. Your primary case manager coordinates every visit and stays current on your status whether or not they personally attend each one.

Routine skilled nursing visits are scheduled Monday–Friday during standard business hours. Visits can be scheduled on weekends and evenings when medically necessary per the physician's plan of care. Kassy Health maintains a 24/7 on-call clinical line for current patients with urgent questions between visits.
