Medical social work at home

Medical social work addresses the human context of illness — the family stress, financial strain, community isolation, and uncertainty about what comes next. A licensed medical social worker is a core member of the Kassy Health care team, visiting the patient at home to connect families with resources, provide counseling, and help navigate the healthcare system at a time when that can feel overwhelming.

Clinical Scope

What our medical social workers do

  • Caregiver counseling and support — addressing burnout, grief, and stress
  • Advance care planning — healthcare proxy, living will, DNR conversations
  • Long-term care planning — assisted living, memory care, hospice transitions
  • Housing and safety concerns — referrals when home environment is unsafe
  • Mental health resources and referrals — depression, anxiety, grief after diagnosis
  • Care coordination — communication between home health team, physicians, and family
For Family Caregivers

The social worker is for you, too

Family members caring for a loved one at home often shoulder an enormous burden — physical, emotional, and financial. Caregiver burnout is real, and it directly affects patient outcomes. Our medical social workers create space to acknowledge this honestly.

The social work visit is not just a checklist. It's a conversation. We will listen to what isn't working, identify where the system can help, and follow through on connecting your family with real resources — not just a list of phone numbers.

Common Situations We Help With

You don't have to figure this out alone

"We don't know how to pay for this."

Medicaid planning, prescription assistance, local charity programs — we know what's available in Central Florida.

"My parent lives alone and I'm worried."

Safety assessments, meal delivery, check-in programs, emergency response systems — coordinated by your social worker.

"We need to talk about what comes next."

Advance care planning, hospice conversations, memory care — we facilitate these conversations with care and experience.

"I'm exhausted and can't keep doing this."

Respite care options, caregiver support groups, mental health referrals — caregiver wellbeing is part of patient care.

Cost & Coverage

What does medical social work cost?

For eligible homebound patients, Medicare Part A covers 100% of approved medical social work visits — no copay, no deductible — when social work is ordered as part of an active home health plan of care that includes a qualifying skilled service (skilled nursing, PT, or speech therapy).

Medical social work is not a separate benefit — it is included within the Medicare home health episode. If you are already receiving skilled nursing or therapy from Kassy Health, ask your care team about adding social work to your plan of care if you have needs in any of the areas listed above. There is no additional cost.
