What the first 72 hours after discharge look like

The transition from hospital to home is the highest-risk window for surgical patients. Kassy Health's care coordination begins before the patient leaves the hospital.

  • A Kassy Health nurse contacts the family before discharge to confirm the care plan, review equipment needs, and answer questions.
  • The first nursing visit occurs within 24–48 hours of arriving home — not days later.
  • The nurse reviews all discharge instructions, reconciles medications, assesses the wound or incision, and identifies any early complications before they escalate.
  • Therapy evaluations typically happen in days 2–4 depending on surgeon orders and patient tolerance.
  • The surgeon remains the clinical authority — we report back directly to the surgeon's office and escalate concerns immediately.

Same-day intake available. When a referral is received early in the day, Kassy Health can often complete intake and schedule the first nursing visit for the same day of discharge.

Knowing what to watch for between nursing visits can prevent a complication from becoming an emergency. Share this list with all family members and caregivers.

  1. Wound changes

    Wound edges opening, excessive bleeding, or foul-smelling drainage are signs of dehiscence or infection. Call the care team or go to the emergency room immediately.

  2. Elevated temperature

    A temperature above 101°F (or as directed by the surgeon on discharge paperwork) is a potential sign of post-surgical infection. Report immediately.

  3. Worsening incision site

    Increased redness, warmth, or swelling around the incision beyond the first two days warrants a call to the care team — not a wait-and-see approach.

  4. Chest pain or sudden shortness of breath

    These symptoms can indicate a pulmonary embolism — a blood clot in the lungs — which is a serious post-surgical risk. Call 911 immediately.

  5. Calf pain, warmth, or swelling

    Deep vein thrombosis (DVT) is common after joint replacement and major surgeries. Calf pain or a swollen, warm lower leg needs same-day evaluation. Call the care team immediately.

Does Medicare cover home health after surgery?

Medicare covers post-surgical home health when the patient meets the following criteria:

  • The patient is homebound — meaning leaving the home requires a considerable and taxing effort.
  • The patient requires skilled nursing or therapy — not only custodial or personal care assistance.
  • A physician or surgeon certifies the need and signs the plan of care.
  • Care is delivered by a Medicare-certified home health agency — which Kassy Health is.

Most major surgeries qualify. Coverage includes wound care supplies used during skilled nursing visits. An inpatient hospital stay is not required — outpatient surgery patients may also qualify.

No cost for most patients. When Medicare is the primary payer and all eligibility conditions are met, there is typically no out-of-pocket cost for covered home health services under Medicare Part A.

Yes. That is not a problem. Kassy Health works with surgeons across Central Florida regardless of hospital affiliation. We need a signed plan of care and Medicare certification from your surgeon. Our intake team handles the paperwork coordination directly with the surgeon's office — families do not need to manage this themselves.

Yes. Medicare does not require an inpatient hospital stay to qualify for home health. If you are homebound and your surgeon certifies a need for skilled nursing or therapy after outpatient surgery, you may qualify for Medicare-covered home health services. Common examples include same-day joint replacement and laparoscopic procedures where the patient goes home the same day.
