The clinicians caring for your patients.
When you refer to Kassy Health, your patient is seen by a coordinated, multidisciplinary team — skilled nurses, therapists, and social workers who keep you, the referring physician, informed at every stage of the plan of care.
The disciplines on every plan of care
Each patient's plan of care draws on the disciplines their condition requires — coordinated under one agency so communication stays consistent and your orders are carried out faithfully.
Clinical
Skilled Nursing (RN/LPN)
Registered and licensed practical nurses provide wound care, medication management, disease education, IV therapy, and ongoing assessment — escalating to you when a patient's status changes.
Therapy
Physical Therapy
Therapists restore strength, balance, and safe mobility in the home, build individualized exercise programs, and reduce fall risk after surgery, stroke, or hospitalization.
Therapy
Occupational Therapy
OTs help patients regain independence in daily activities — dressing, bathing, and self-care — and recommend adaptive equipment and home modifications.
Therapy
Speech-Language Pathology
Speech therapists address swallowing safety, communication, and cognitive-linguistic recovery — common after stroke, neurological events, or prolonged hospitalization.
Support
Medical Social Work
Medical social workers connect patients to community resources, coordinate benefits, support advance care planning, and address the social barriers that drive readmissions.
Support
Home Health Aide
Aides assist with personal care and daily living under the supervision of the skilled team, providing the hands-on support that keeps patients safe and comfortable at home.
How we coordinate with you
Referring physicians stay at the center of the plan of care. Our intake and clinical teams are built around keeping you informed without adding to your administrative load.
Same-day intake calls
Our intake coordinator confirms eligibility and insurance the same day a referral arrives, so care can begin without delay.
Plans of care returned within 48 hours
The signed plan of care (CMS-485) is prepared and returned to your office within 48 hours of the initial nursing evaluation.
Proactive communication on status changes
When a patient's condition changes, the assigned nurse contacts your office directly — not a phone tree — with the clinical detail you need to act.
OASIS-driven assessments
Every episode is grounded in a thorough OASIS assessment, ensuring your orders are matched to the patient's documented functional and clinical status.
Licensed clinicians, accountable oversight
Every clinician on the Kassy Health team is licensed or certified in their discipline and works under the supervision of our clinical leadership. We hold our work to the standards Medicare and our accreditors require.
